一例性别认同障碍的心理动力学咨询案例

• 摘要： 本案例是典型的后天成长环境等因素引发的性别认同障碍。来访者出生在母亲主导，父亲缺失话语权的原生家庭模式中。来访者仅完成了生理上的性别确认，心理上的性别认同发生困难。案例中采用心理动力学咨询方案，重点在于识别来访者的内在心理冲突、防御机制、创伤体验等。通过建立治疗联盟、觉察移情与反移情、倾听和会谈技术等，梳理来访者内在冲突来源。通过咨询， 来访者对自我身份和性别认同有了更深探索，内在冲突减少，痛苦感减轻。
• This case is a typical gender identity obstacle caused by factors such as acquired growth environment. The visitor was born in the original family model dominated by the mother and the father lacked the right to speak. The visitor only completed the physiological gender confirmation, and the psychological gender identity was difficult. In the case, the psychodynamic counseling scheme is adopted, focusing on identifying the visitors’ internal psychological conflict, defense mechanism, traumatic experience, etc. Through the establishment of treatment alliance, awareness of empathy and anti-empathy, listening and talking technology, sort out the sources of internal conflicts of the visitor. Through consultation, the visitor s has a deeper exploration of self-identity and gender identity, and internal conflicts and pain reduced.
